Consent Forms

Annual consent forms and third-party service information for SIDE parents and caregivers.

The consent forms below should be completed by parents/caregivers at the commencement of each school year. The forms provide opportunities to indicate consent for media, television/film ratings and third-party services used in teaching and learning at SIDE.

Primary K-6 and Languages Complete the 2026 consent form for Primary K-6 and Primary Languages. Secondary Years 7-12 Complete the 2026 consent form for Secondary Years 7-12.

Starting in 2027?

Primary K-6 and Primary Complete the 2027 Primary K-6 and Primary Languages consent form. Secondary Years 7-12 Complete the 2027 Secondary Years 7-12 consent form.
